Change Schools - documents & resources for schools in receipt of funding.

The documents below describe the planning and evaluation process that you will use as a Change School;

Throughout the programme, Change Schools will record their planning and evaluation on Creative Partnerships Projects Database. The list below outlines the forms that have to be completed:

  • Change Schools CSDF Planning Form
  • Project planning form
  • Project evaluation form – mid-point
  • Project evaluation form – end-point
  • Project end form
  • Session recording form – (this is optional and it is not completed on the CPPD – see the Planning and evaluation guidance for more information)

School Coordinators and Creative Agents are responsible for completing the forms online on the Creative Partnerships Projects Database which can be found at
https://creativeweb.creative-partnerships.com. Guidance can also be found on the Database in the ‘help’ section.

Your local Area Delivery Organisation will be able to advise you on how to use this system.
